ABSTRACT

Objective:To evaluate the ability of light dispersive colorimetry to detect hemoglobin (Hb) in lipid blood samples and its feasibility as an alternative to plasmapheresis commonly used in the laboratory.Methods:Routine blood samples of 276 inpatients in Fujian Provincial Hospital from July 2020 to July 2021 were collected. Routine blood samples of 276 inpatients were collected. There were 169 males and 107 females, aged from 16 to 97 years. 183 non-lipid blood samples and 93 lipid blood samples were collected.(1) One case each of low, medium and high Hb value in non-lipid blood and lipid blood samples were collected, and the precision and the linearity of light scattering method was detected.(2)Non-lipid blood samples divided into Hb low-value group, median-value group and high-value group, which were measured by light scattering method and colorimetric method to compare Hb values. (3)Non-lipid blood samples were divided into Hb low-value group, median-value group and high-value group. Plasma exchange was carried out with different concentrations of fat emulsion. The bias and linearity of Hbc2 and Hbc1, Hbo2 and Hbc1 were analyzed by MedCalc19.1 software. The Hbc2 and Hbc1 bias ( CV%) and Hbo2 and Hbc1 bias ( CV%) were calculated. T test was used to analyze the influence of different concentrations of triglyceride on Hb bias.(4)Blood samples were divided into Hb low-value group, median-group and high-value group. The Hb of light scattering method was compared with the colorimetric method after plasma exchange. Results:(1)The intrabatch precision of light scattering method for non-lipid blood and lipid blood specimens was within the allowable range ( CV<1.5%), and the good linearity ( R2=1.000).(2)The bias of Hb measured by light scattering method and colorimetric method in the three groups was below 3.5%(-0.58±2.34,0.16±1.52,1.15±1.56), within the allowable total error range. The two methods have the equivalence and good linear relationship ( r=0.999).(3)The concentrations of Hbo2 in the low (except 4.1 mmol/L), medium and high Hbo2 groups were equivalent to those in the non-lipid blood colorimetry (Hbc1), and the two methods were well correlated. The results of light scattering method have nothing to do with the concentration of lipid blood.(4)There was no significant difference of the Hb between the light scattering method and plasma exchange method in three groups ( P>0.05), Both of them have equivalence and good correlation ( R2=0.968,0.948,0.870). Conclusion:Light scattering method can effectively reduce the effect of lipid blood on hemoglobin determination, and can replace the traditional plasma exchange method, which has high clinical application value.

ABSTRACT

Objective To discuss the relationship between discipline ranking and personnel rationality,and explore the problems of personnel development among all the disciplines and put forward further countermeasures.Methods A 3A general hospital in Beijing was targeted,and departments in it were divided into,in descending order of hospital regulations,key disciplines,developing-key disciplines,characteristic disciplines and general disciplines.To analyze the rationality of personnel,four dimensions as educational background structure,professional title structure,age structure and academic proficiency of department leader were adopted.Results The hospital had 52 departments in total,which were divided into key disciplines (7),developing-key disciplines (10),characteristic disciplines (6) and general disciplines (29).With the decline of the discipline level,the problem of the talent team was gradually increasing.The major problem of personnel among all the disciplines was that the proportion of senior staff was too high,so did the key disciplines and developing key disciplines.Age structure irrationalized was another major problem for developing-key disciplines.As for characteristic disciplines,the problems in personnel were even diversified,which included not only the irrational age structure and senior professional proportion,but also the fact that director of one third of these departments showed low academic proficiency.In general disciplines,all above four types of problems exited.Conclusions The rationality of the talent team is closely related to the level of the discipline.Problems in personnel varied among different disciplines,and countermeasures should be provided targeted at existing problems.

ABSTRACT

Objective To observe the clinical effects of magnified pedicle subtraction osteotomy (mPSO) via the posterior approach combined with biomimetic bone graft fusion and internal fixation for the treatment of thoracolumbar Kummell's disease and kyphosis.Methods A total of 11 patients with thoracolumbar Kummell's disease and kyphosis deformity were treated at our department from March 2012 to June 2016.They all underwent mPSO via the posterior approach combined with biomimetic bone graft fusion and internal fixation.They were 3 men and 8 women with an average age of 61.2 years.The clinical effects were evaluated according to visual analogue scale (VAS),Japanese Orthopaedic Association (JOA) scoring,American Spinal Injury Association (ASIA) grading,cobb angle correction and rate of bone graft fusion at preoperation and 2 weeks,1,3,6 and 12 months after operation.Results All the patients were followed up for an average of 11.4 months (from 10 to 13 months).The VAS scores (2.1 ±0.5 points and 1.1 ±0.2 points),JOA scores (23.6 ±3.8 points and 25.5 ±3.2 points) and cobb angles (8.1°± 1.5° and 13.8°±2.1°) at 2 weeks after operation and final follow-ups were significantly improved from their preoperative values (8.1 ± 0.6 points,12.1 ± 3.6 points and 51.3° ± 9.8°,respectively) (P < 0.05).However,there were no significant differences between 2 weeks after operation and the final follow-up in terms of the above values (P > 0.05).The ASIA grading was improved from preoperative grade C to postoperative grade D in one case,and from preoperative grade D to postoperative grade E in 3 cases.At final follow-ups,bony fusion was observed at all the bone graft sites,with a fusion rate of 100%.Conclusion mPSO via the posterior approach combined with biomimetic bone graft fusion and internal fixation is a good treatment for patients with thoracolumbar Kummell's disease and kyphosis deformity.

ABSTRACT

Objective To investigate the effects of artificial airway cuff pressure at different monitoring intervals on ventilator associated pneumonia (VAP) in critical patients. Methods About 56 patients treated with mechanical ventilation from January 2015 to December were grouped by ward numbers:32 patients in the south ward were in the experiment group and 24 in the north ward were in the control group. The two groups were given the same routine care, e.g., mechanical ventilation by endotracheal intubation. In the experiment group, the cuff pressure was controlled to 30cm H2O every six hours and the control group was every 4h. The two groups were compared in terms of the incidence of VAP during mechanical ventilation, occurrence time of VAP, duration of mechanical ventilation and mortality. Result The incidence of VAP during mechanical ventilation, occurrence time of VAP, duration of mechanical ventilation, and mortality showed no significant differences between the two groups (P>0.05). Conclusions For mechanically ventilated patients, the cuff pressure controlled to 30cm H2O every 6h can not increase the incidence of VAP, duration of mechanical ventilation and mortality. It can reduce the nursing workload.

ABSTRACT

Objective To observe the effects of electroacupuncture at Ganshu (BL18) and Zusanli (ST36) on depression state and gastrointestinal motility and relevant protein expressions in brain of rats with stress-induced depressive functional dyspepsia. Methods Totally 48 Wistar rats were randomly divided into normal, model, electroacupuncture (EA) and sham-operation group, with 12 rats in each group. Depressive functional dyspepsia model was established by chronic unpredictable mild stress protocols. From day 15 to day 28, the EA group was given electroacupuncture at BL18 and ST36, and the sham-operation group was stimulated by electroacupuncture at 1/3 and 2/3 of the tail. Body weight test, open field test and sucrose preference test were measured on day 0, day 14 and day 28. After 14-day administration, gastric residual volume and intestinal propulsion rate of semisolid paste were detected. The expression of βCaMK inⅡ lateral habenulas and BDNF in hippocampus were measured by Western blot. Results Compared with the normal group, the weight, the sucrose preference rate, the behavior score, the small intestinal propulsion rate of semisolid paste and the expression of BDNF in hippocampus decreased significantly both in the model group and sham-operation group (P<0.01), and the gastric residual volume of semisolid paste and the expression of βCaMK inⅡ lateral habenulas increased significantly (P<0.01). Compared with the model group, the weight, the sucrose preference rate, the behavior score, the small intestinal propulsion rate of semisolid paste and the expression of BDNF in hippocampus increased significantly in EA group (P<0.01), and the gastric residual volume of semisolid paste and the expression of βCaMK inⅡ lateral habenulas decreased significantly (P<0.01). Compared with the sham-operation group, the expression of βCaMKⅡ in lateral habenulas decreased significantly, the expression of BDNF in hippocampus increased significantly in EA group (P<0.01). Conclusion Electroacupuncture at Ganshu (BL18) and Zusanli (ST36) can improve depression and gastrointestinal motility of rats with depressive functional dyspepsia, which may be related to the inhibition of βCaMK inⅡ lateral habenulas and the activation of BDNF in hippocampus dyspepsia.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effects of direct current electric fields on directional migration and arrangement of dermal fibroblasts in neonatal BALB/c mice and the related mechanisms.</p><p><b>METHODS</b>Twelve neonatal BALB/c mice were divided into 4 batches. The skin on the back of 3 neonatal mice in each batch was obtained to culture fibroblasts. Fibroblasts of the second passage were inoculated in 27 square cover slips with the concentration of 5 × 10(4) cells per mL. (1) Experiment 1. Six square cover slips inoculated with fibroblasts of the second passage were divided into electric field group (EF) and sham electric field group (SEF), with 3 cover slips in each group. The cover slips were put in live cell imaging workstation. The cells in group EF was treated with electric power with EF intensity of 200 mV/mm, while simulating process without actual power was given to SEF group (the same below) for 6 h. Cell proliferation rate was subsequently counted. (2) Experiment 2. Six cover slips were divided and underwent the same processes as in experiment 1. Cell movement locus within EF hour (EFH) 6, direction change of cell migration at EFH 0 (immediately), 1, 2, 3, 4, 5, and 6 which was denoted as cos(α), cell migration velocity within EFH 6, direction change of long axis of cell within EFH 6, and direction change of cell arrangement at EFH 0, 1, 2, 3, 4, 5, and 6 which was denoted as polarity value cos[2(θ-90)] were observed under live cell imaging workstation. After EFH 6, the morphological changes in microtubules and microfilaments were observed with immunofluorescent staining. (3) Experiment 3. Six cover slips were divided into cytochalasin D group (treated with 1 μmol/L cytochalasin D for 10 min) and colchicine group (treated with 5 μmol/L colchicine for 10 min), with 3 cover slips in each group. The morphological changes in microfilaments and microtubules were observed with the same method as in experiment 2. (4) Experiment 4. Nine cover slips were divided into control group (no reagent was added), cytochalasin D group and colchicine group (added with the same reagents as in experiment 3), with 3 cover slips in each group. Cells in the 3 groups were exposed to an EF of 200 mV/mm for 6 h. Cell movement locus within EFH 6, cell migration velocity within EFH 6, cell polarity values at EFH 0, 3, and 6, and morphological changes of cells at EFH 0 and 6 were observed. Data were processed with independent samples t-test, one-way analysis of variance, and LSD test.</p><p><b>RESULTS</b>(1) There was no statistically significant difference in cell proliferation rate in group EF and group SEF (t=-0.24, P﹥0.05). (2) Within EFH 6, cells in group EF migrated towards the anode of EF, while cells in group SEF moved randomly. At EFH 0, the values of cos(α) of cells in the 2 groups were both 0. The absolute value of cos(α) of cells in group EF (-0.57 ± 0.06) was significantly higher than that in group SEF (0.13 ± 0.09, t=6.68, P<0.01) at EFH 1, and it was still higher than that in group SEF from EFH 2 to 6 (with t values from 5.33 to 6.83, P values below 0.01). Within EFH 6, migration velocity of cells in group EF was (0.308 ± 0.019) μm/min, which was significantly higher than that in group SEF [(0.228 ± 0.021) μm/min, t=-2.76, P<0.01]. Within EFH 6, long axis of cells in group EF was perpendicular to the direction of EF, while arrangement of cells in group SEF was irregular. Cell polarity values in group EF were significantly higher than that in group SEF from EFH 2 to 6 (with t values from -7.52 to -0.90, P values below 0.01). At EFH 6, the morphology of microfilaments and microtubules of cells in EF group was similar to that in SEF group. (3) The fluorescent intensity of microfilaments of cells in cytochalasin D group became weakened, and the filamentary structure became fuzzy. The microtubules of cells in colchicine group became fuzzy with low fluorescent intensity. (4) Within EFH 6, cells in control group migrated towards the anode of EF, while cells in cytochalasin D group and colchicine group moved randomly. Within EFH 6, there was statistically significant difference in migration velocity of cells in the 3 groups (F=6.36, P<0.01). Migration velocity of cells in cytochalasin D group and colchicine group was significantly slower than that in control group (P<0.05 or P<0.01). At EFH 0, 3, and 6, cell polarity values in the 3 groups were close (with F values from 0.99 to 1.51, P values above 0.05). At EFH 0, cells in control group were spindle; cells in cytochalasin D group were polygonal or in irregular shapes; cells in colchicine group were serrated circle or oval. At EFH 6, no morphological change was observed in cells in control group; cells in cytochalasin D group were spindle with split ends on both ends; cells in colchicine group were serrated oval.</p><p><b>CONCLUSIONS</b>The physiologic strength of exogenous direct current EF can induce directional migration and alignment of dermal fibroblasts in neonatal BALB/c mice. Microfilaments and microtubules are necessary skeleton structure for cell directional migration induced by EF, while they are not necessary for cell directional arrangement induced by EF.</p>

ABSTRACT

BACKGROUND:Hematopoietic stem cel transplantation has therapeutic effects on many diseases, but its application has some limitations, such as cel harvesting and age-limited number of cels. OBJECTIVE: To investigate the application value of bone marrow mesenchymal stem cels in hematopoietic stem cel transplantation for sensitized and non-sensitized BALB/c mice. METHODS: Bone marrow cels derived from BALB/c mice were isolated and culturedin vitro to harvest mesenchymal stem cels using adherent method. The cel surface markers were detected by flow cytometry. A murine model of sensitization was established by transfusion of alogeneic spleen cels. Mesenchymal stem cels labeled with green fluorescent dye were transplanted into non-sensitized and sensitized recipient mice, and the homing of mesenchymal stem cels in vivowas monitored at different time points post transplantation. Additionaly, under irradiation pretreatment, sensitized BALB/c mice under irradiation were subjected to combined transplantation of alogeneic bone marrow cels and syngeneic mesenchymal stem cels. Survival rate of BALB/c mice was monitored daily. RESULTS AND CONCLUSION: At 48 hours after transplantation, mesenchymal stem cels in sensitized and non-sensitized recipients were homing to the spleen and bone marrow, respectively. In the experiment of hematopoietic stem cel transplantation, the sensitized recipients died at 12-15 days after combined transplantation, with a median of 14 days; however, the sensitized recipients only undergoing alogeneic bone marrow cel transplantation had a survival median of 13 days. These findings indicate that the transplanted mesenchymal stem cels in sensitized recipients are mainly homing to the spleen and bone marrow, but the combination transplantation cannot enhance the transplantation of alogeneic hematopoietic stem/progenitor cels in sensitized recipients.

ABSTRACT

Objective To investigate the clinical pathological factors of papillary thyroid microcarcinoma (PTMC) with clinically node-negative sides of neck stage central lymph node metastasis.Methods Analysis of 136 cases of cNo stage of PTMC of clinical and pathological data,and using the PCR method for the detection of BRAFV600E mutation in the paraffin coated tissue.Results One hundred and thirty-six cases with PTMC lymph node metastasis rate was 38.2% (52/136),BRAFV600E mutation rate was 44.9% (61/136).Single factor analysis showed that the lymph node metastasis was related with BRAFV600E mutation and capsular invasion (P< 0.05).The size of the tumor was close to significant level (P=0.057).Multiple regression analysis showed that BRAFV600E mutation and capsular invasion were independent factors for affecting the central lymph node metastasis (P < 0.05).Conclusion The routine central lymph node dissection should be performed in patients with BRAFV600E mutation and tumor invasion.

ABSTRACT

Objective To establish and validate a standard method using flow cytometry (FCM)in human ABO blood group antibody (Ab) detection. Methods Sera samples from 52 blood donors were incubated with standardize red blood cells (RBCs), and anti-A/B antibody (IgM/IgG) was measured by indirect flow cytometry after adding secondary isotype-specific fluoresce labeled Ab. The results were compared with those by using ELISA reader. Results The changes of anti-A/B IgM measured by both methods correlated well and an overall correlation coefficiency of 0.730 for IgM was obtained by Spearman's rank testing (P<0.05). FCM showed better specificity, sensitivity and reproducibility over ELISA reader. In blood group A samples (n = 16) or B samples (n = 16) ,the antirespectively. IgG blood group Ab could only be detected in blood group O sera. Conclusion Compared with traditional methods,FCM is a more objective method to offer accurate detection of natural ABO blood group Ab (IgM/IgG) and allowed semi-quantitative measurement of antibody levels.

ABSTRACT

Objective To explore the nursing strategies for preoperative ventilator-dependent patients undergoing discontinuing mechanical ventilation after lung transplantation.Methods The clinical data and nursing experiences of 10 preoperative ventilator-dependent patients undergoing discontinuing mechanical ventilation after lung transplantation from September 2002 to December 2008 were retrospectively analyzed.Results The time of discontinuing mechanical ventilation ranged from 7 to 30 days and the average time was 16.5 days.Conclusion The key points to ensure the success of discontinuing mechanical ventilation were careful observation of ventilation modes,preparations before discontinuing,airway management,nutritional support and psychological nursing.

ABSTRACT

Objective To investigate whether cotransplant with xenogenetic neonatal porcine Sertoli cells (NPSCs) could prolong rat islet allograft survival and its mechanisms.Methods 1500 islets equivalent quantity (IEQ) and 1×10~7 NPSCs were implanted under renal capsule of diabetic Wistar rats.Islets implanted alone were used as control group (n=6);islets co-transplanted with NPSCs under left renal capsule of recipients served as experimental group (n=6);meanwhile,islets and NPSCs implanted into the different sides of kidneys were used as another control grouP(n=4).Blood glucose level was measured everyday.The graft-bearing kidneys at the time of rejection were Results Co-transplantation with NPSCs to the same site significantly prolonged islet allograft survival (mean survive time,16.3±1.4 days vs.5.7±1.0 days in islet transplant alone control group,P<0.05).In contrast,transplantation with NPSCs and islets separately did not prolong the islet allograft survival (5.3±0.5 days).HE staining showed plenty of local infiltrated lymphocytes in the transplanted site of the eontrol group.which were demonstrated as mainly CD3+ T cells by immunopathology.The local expression of Bcl-2 was markedly elevated in co-transplantation group as compared with the other 2 groups,while there were no significant differences in the HO-1 expression among these groups.Conclusion Co-transplantation with xenogenic NPSCs can significantly prolong islet allograft survival in rats.The immunoprotective mechanism may be associateel with the inhibition of lymphocyte infiltration and the enhancement of the local expression of protective gene Bcl-2.

ABSTRACT

Objective:To study the master iodine nutrition status of population living in areas with different iodine in drinking water and the cut-off point of stopping iodized salt supply and the range of adequate iodine intake for the strategy of prevention from iodine deficiency or excess. Method:Thirteen townships points along the Yellow River downstream in Shandong were investigated and divided into six groups(A-F) based on water iodine concentrations of 0-50,50-100,100-150,150-300,300-800 and over 800 ?g/L. The indicators of water iodine,edible salt iodine,urinary iodine(UI) and thyroid goiter were observed. Results:From A,B,C,D,E and F group,the medians of water iodine concentration were 20.3,91.4,143.2,203.6,341.9 and 812.3 ?g/L,of edible salt iodine were 0,25.7,25.8,30.4,36.4 and 33.0 mg/kg,and of UI were 116.8,354.2,400.4,607.9,881.3 and 1 213.8 ?g/L respectively. The goiter rates were 10.8,8.6,15.0,14.2,14.9 and 25.0 % respectively . The sample proportion of UI below 100 ?g/L was 8.2%,of 100-300 ?g/L was 18.2% and of over 300 ?g/L was 73.6%. Frequency excursion of UI had a high trend towards water iodine concentration over 90?g/L in 5 groups. The proportion of UI over 300 ?g/L increased,consistent with higher water iodine level. Median UI of all groups with iodized salt decreased significantly,especially group F. There were significant correlations between UI and water iodine. Iodine nutrition status was markedly excessive in group B,C,D,E and F. Conclusions:Iodine nutrition status of most residents consuming drinking water with iodine concentration about 20 ?g/L should be adequately supplied with eligible iodized salt. Iodine intakes in groups B-F were excessive,and iodized salt supply should be stopped there. The cut-point of water iodine level where iodized salt stopped could possibly be 90 ?g/L,and the range of adequate iodine intake,in terms of drinking water,could be defined at range of over 20 to 90 ?g/L in iodine concentration.

ABSTRACT

Objective To study the effects of supplying iodine on thyroid autoimmunity in iodine deficiency subjects. Methods The levels and abnormal rates of immunoglobulins (IgA, IgG, IgM), complement C 3, and thyroid autoantibodies such as thyroid peroxidase antibody (TPOAb), thyroglobulin antibody (TGAb), thyrotropin receptor antibody (TRAb) and thyroid stimulating antibody(TSAb) were observed in inhabitants in iodine deficiency area before and after oral iodized oil capsules were administered. Results The levels of IgA, IgG and C 3 increased after iodine supplying for three months, the proportions of IgG and C 3 which exceeded upper limit of the normal range reached to 38..9% and 62.0% respectively. The levels of IgA and IgG began to decrease markedly six months after iodine supplying. The levels of TPOAb, TGAb and their positive rates after iodine supplying were all higher than those before administration, and reached the highest level between three to six months after iodine supplement. And 7 cases out of 79 or 64 subjects with 2 kinds of positive autoantibodies were observed 3 or 6 months after iodine supplying respectively. The cases were all adult women with the average age of 27.6 yr, and had no clinical symptoms. The levels of TRAb and TSAb after iodine supplying were much higher than that before iodine supplying. The rate of positive TRAb was increasedmarkedlyandreachedto24.4% twelve months later. Conclusion Iodine supplying for the subjects with iodine deficiency seems to induce immune reaction, increase the levels of autoantibodies, and might develop autoimmune thyroid diseases.
